State Employee Wellness Program: STATE OF HEALTH

STATE OF HEALTH is a transformational program designed to improve the health of state employees. Offered at no cost only to employees enrolled in one of the State’s medical insurance plans, this program is designed to support everyone, from healthy individuals with no risk factors to those living with one or more chronic conditions.

All participants are eligible for a $20 monthly premium discount when they complete the simple steps of their Action Plan by the start of the next application period. Learn more about the timing of the incentive program.

Eligibility: Employees are only eligible to participate if they are currently enrolled in one of the State's medical plans through Cigna or Kaiser Permanente. Spouses of participants are not eligible at this time.

The Fitness Academy Personal Training Program

The STATE OF HEALTH Fitness Academy is a new component of the STATE OF HEALTH health coaching program that connects you with a personal trainer who will help you elevate your personal exercise journey. 

Fitness Academy teaches you how to perform specific exercises safely and effectively, beginning with eight personal training sessions (either virtual or in-person at select locations) for the first 12 weeks. Participants are provided with a home exercise kit and have the opportunity to earn milestone rewards throughout the program.

To be eligible you must be enrolled in the State’s health insurance through Cigna or Kaiser Permanente, have one or more risks for chronic conditions, and participate (or have already completed) in the STATE OF HEALTH health coaching pathway. This program is paid 100% by the State of Colorado with zero cost to you. Go to www.coloradostateofhealth.com to enroll and take back control of your well-being.

Member Wellness Tools from Insurance Carriers

If you are enrolled in a State plan through Cigna or Kaiser Permanente, you are encouraged to participate in STATE OF HEALTH to access a number of free resources such as health coaching, personal training, and health and wellbeing programs. If you aren't in STATE OF HEATLH, you can still access additional wellness tools and services through the State's partnered insurance carriers. Cigna

Cigna offers a number of resources for members, including mental health programs, nutritional programs and services, a diabetes prevention program, a price comparison tool (Healthcare Bluebook), and more. Kaiser Permanente

Kaiser Permanente provides access to mental health and wellness support to members. Employees can access a number of self care apps and work one on one with wellness coaches. Kaiser also offers fitness resources such as ClassPass, One Pass Select Affinity, and more. For a full list of available programs and perks, log in to kp.org or visit mybenefits.kp.org/soc

Meet Your Onsite Health Coaches 

Our onsite health coaches offer personal, one-on-one coaching to help improve your overall health – physically and emotionally. Working together, you and your coach can create a plan to help you meet your personal health and lifestyle goals. You must be enrolled in Kaiser Permanente or Cigna to be eligible.

You can talk about almost anything health related, such as reducing stress or quitting tobacco; nutrition, exercise and weight loss; and health conditions such as improving high blood pressure and high cholesterol.   

Your Personal Health Information (PHI) is protected by the Health Insurance Portability and Accountability Act (HIPAA) and the Health Coach “Code of Conduct.” Your managers and coworkers cannot access your PHI.

Katt Adamo, NBC-HWC

A National Board-Certified Coach, Katt began with a bachelor’s in communications from Eckerd Collage later venturing into healthcare. Her love for coaching emerged from substance use disorder program research and an internship in Boston, MA, during her Post-Baccalaureate Pre-Medical studies at Harvard University Ext. Additionally, she is a Mental Health First Aid Instructor and First Aider. With over 5 years of coaching experience, Katt is passionate about empowering customers to lead healthier lives.

An adventurer at heart, she has explored over 30 countries and lived in more than 10 cities. Her most inspiring experiences include building a clinic in Ghana and a school in rural India, which fueled her desire to help others.  

Katt is committed to promoting healthy lifestyles that feel attainable even on the go and under stress! She is eager to make a positive impact across the beautiful state of Colorado!

Candace Steele, MPH, BSN, RN

Education:

  • Bachelor of Science in Nursing from Regis University
  • Master of Public Health from the University of Colorado Denver
  • Bachelor of Science in Health from SUNY Cortland

Candace supports the STATE OF HEALTH program and assists with onsite biometric screenings and health coaching. Candace has career experience as a nurse with various clinical experiences including in medical-surgical, public health, and community and behavioral health. She has spent over 12 years in healthcare and social services.
