Application Checklist for the Colorado Supplier Diversity Directory
The Colorado Supplier Diversity Directory provides businesses with increased visibility to buyers through free directory listings. Any business registered and in good standing with the Colorado Secretary of State can apply. Small, diverse-owned, veteran-owned businesses and businesses located in HUBZones can increase their credibility by verifying their business size and ownership with the Colorado Supplier Diversity Navigator.
If your business is approved, you’ll receive:
- Increased visibility to buyers across the state through a free directory listing
- Logos to use on promotional materials (verified businesses only)
- Increased business credibility by verifying your business size and ownership (for qualifying small, diverse-owned, veteran-owned businesses and businesses located in HUBZones)
- Notifications about upcoming trainings and events offered by the State of Colorado
Applications are currently being accepted and the directory is scheduled to be made public in early April.
To complete your business application and verification, we recommend having the following information and documentation:
Contact Information
- First and last name
- Phone number
- Email address
Business Information
- Business name as registered with the Colorado Secretary of State
- Doing business as (if different from the registered name)
- Business address and website
Information on Goods or Services Offered

Small Business Size Declaration
(for small businesses only)
- Designate your North American Industry Classification System (NAICS) commodity code(s)
- List all that apply

- Small business size standard for your business using the SBA Size Standards Tool
- If applying for a small business verification, total average revenue for your company and its affiliates over the past three years
- Tax documents must be submitted to receive verified small business designation
- Select a category based on small business size
Verifications
Decide which verifications you’re applying for and gather the required documentation. For more information on certifications, visit our

HUBZone Verification
- HUBZone certification from the Small Business Administration, OR
- Provide all business address locations in and out of Colorado HUBZones and include a detailed description of the work performed at each location specifying the number of employees at each location and the percentage of work performed at each location (percentages should add to 100%)
Small Business Verification
- Documentation of your small business certification that you have gone through a third-party verification to determine business size (can’t be a self-identified certification) OR
- Tax filings for the past three or five years redacting social security numbers (you will calculate total average revenue for these three or five years)
Diverse Business Verification
- Documentation of any third-party business certifications that indicate your business has demonstrated your diverse business ownership (can’t be a self-identified certification)
Veteran Business Verification
- Documentation from a third-party certifying entity that indicates your business has demonstrated your veteran-owned business status (can’t be a self-identified certification)

Memorandum of Agreement for the Colorado Supplier Diversity Directory
You must accept the terms of this Memorandum of Agreement in order to register for a Supplier business listing on the Colorado Supplier Diversity Directory.
You certify that you are duly authorized by the Supplier named in this application to: (i) register the Supplier; (ii) file, on behalf of the Supplier, all of the information requested in this registration process; and (iii) enter into this Memorandum of Agreement on behalf of the Supplier. By submitting this electronic directory registration, you hereby agree on behalf of the Supplier and for the benefit of each State agency and public body that:
- The Supplier is registered and in good standing with the Colorado Secretary of State.
- The Supplier will use the Colorado Supplier Diversity Directory registration update form to update the Supplier's registration information whenever necessary to ensure that the registration information remains accurate, complete and current at all times.
- The Supplier hereby warrants that the information provided by the Supplier through the Colorado Supplier Diversity Directory registration form and registration update form shall at all times be accurate, complete and current, and to maintain the Supplier’s “verified” status (if applicable).
- The Supplier provides the State of Colorado with consent to make information available on the Colorado Supplier Diversity Directory (public directory) and releases the State of Colorado from any unintended harm or consequences as a result of the Supplier’s participation.
- The Supplier will contact the Colorado Supplier Diversity Navigator to provide notice of any change of status if at any time during participation in the program the Supplier no longer meets the program requirements for their business designation.
- The Supplier will only use any credentials (logos, verified status references, etc.) offered through the Colorado Supplier Diversity Directory while they are actively participating in the program.
- The Supplier agrees to receive email communications from the State of Colorado.
- The Supplier agrees to publicly display information provided during the registration application process, including Supplier’s business name, goods/services category, “about” information, website, business email address, business address, and verified business status (if applicable).
Listings are offered as a courtesy and no work is guaranteed. Products and services must meet quality, availability and performance standards as determined by the State of Colorado. Any disputes must be timely and reasonably resolved.
This Memorandum of Agreement is between the Supplier and the State of Colorado’s Supplier Diversity Program in the Statewide Equity Office at the Colorado Department of Personnel & Administration.
Participation is voluntary and listings can be removed by the authorized person with appropriate credentials by contacting the Colorado Supplier Diversity Navigator.
Failure to comply with the terms of the Memorandum of Agreement will result in the cancellation of your registration and the removal of the Supplier’s listing from the Directory.
